Codex hook approval is requested again on every agent launch

From moh.sheir14Done 1 day agovia agent-mcp

Opening Codex agents repeatedly shows “Hooks need review — 1 hook is new or changed” after the user has already trusted it. Expected: approve an unchanged hook once and retain approval across agent restarts and launches in the same project. Steps: open a Codex agent, trust the hook, close and reopen the agent or open another Codex agent. Local inspection found the same hook trust hash in several per-agent configs. Those configs state that AgentsRoom rewrites them on every spawn. This suggests config regeneration may discard approval state; that cause still needs confirmation.

Fixed, available in the next version. Each Codex agent launched by AgentsRoom has its own small settings file, and Codex saved your hook approval in that file, which the app rebuilt from scratch at every launch. The approval was lost on restart and the other agents of the project never saw it. Approvals are now moved into the project's shared Codex settings: approve an unchanged hook once and it stays approved for every Codex agent of that project. If you still get the review prompt after this update, please reopen this ticket with as much detail as possible (a screenshot or video helps a lot).
