Load the etiquette by section through an index instead of the whole _etiquette.md at once

Rather than loading the full _etiquette.md file at the beginning of the session, it would be good to load an index.md file which links to the folder locations of separate .md files. There are times when only parts of the file ought to be loaded into context.

This is now in place and will be available in the next version. The etiquette file read at startup is now a short index, and each tool family has its own small file that agents only open right before they first use that tool, so a session start or resume no longer pays for the whole guide. There is also a setting (global, folder, project or agent) to stop agents from reading it at all. If this is not quite what you had in mind, let us know and we will adjust.
